How We Use Information
- To respond to appointment, contact, and referral requests.

We do not sell your information. We do not use Site form data for marketing without your consent.
Protected Health Information (PHI)
Please do not submit detailed medical history or sensitive health information through unsecured web forms. Appointment and contact forms on this Site are for general inquiries only. For clinical communication, use our patient portal or call your location directly. Our referral form for licensed providers uses HIPAA-compliant secure email transmission (Paubox) and does not store PHI on this website.
Third-Party Services
The Site uses Google Analytics 4 for aggregate traffic measurement, Google Maps for location embeds, and Paubox for HIPAA-compliant email delivery of provider referrals. Each of these services has its own privacy policy.
Your Rights
Depending on your state of residence, you may have rights to access, correct, or request deletion of personal information we hold about you. To exercise these rights, contact privacy@axisortho.com.

Complaints
If you believe your privacy rights have been violated, you may file a complaint with our Privacy Officer or with the U.S. Department of Health and Human Services. You will not be retaliated against for filing a complaint.
