Cookies — we use essential cookies for site functionality and analytics cookies (Google Analytics 4) to understand aggregate usage. You may disable cookies in your browser.

How We Use Information

To respond to appointment, contact, and referral requests.
To improve the Site and understand how patients find us.

We do not sell your information. We do not use Site form data for marketing without your consent.

Protected Health Information (PHI)

Please do not submit detailed medical history or sensitive health information through unsecured web forms. Appointment and contact forms on this Site are for general inquiries only. For clinical communication, use our patient portal or call your location directly. Our referral form for licensed providers uses HIPAA-compliant secure email transmission (Paubox) and does not store PHI on this website.

Third-Party Services

The Site uses Google Analytics 4 for aggregate traffic measurement, Google Maps for location embeds, and Paubox for HIPAA-compliant email delivery of provider referrals. Each of these services has its own privacy policy.

Your Rights

Depending on your state of residence, you may have rights to access, correct, or request deletion of personal information we hold about you. To exercise these rights, contact privacy@axisortho.com.

Treatment — to coordinate your care among physicians, nurses, technicians, physical therapists, and other personnel involved in your treatment.
Payment — to bill and obtain payment from you, your insurance company, or a third party.
Health Care Operations — for quality improvement, training, accreditation, licensing, and business management.
As Required by Law — including public health reporting, health oversight, judicial proceedings, and law enforcement.

Your Rights Regarding PHI

Request access to and copies of your medical record.
Request corrections (amendments) to your record.
Request an accounting of certain disclosures we have made.
Request restrictions on how we use or disclose your PHI.
Request confidential communications (e.g., by mail rather than phone).
Receive a paper copy of this notice, even if you have agreed to receive it electronically.
File a complaint if you believe your privacy rights have been violated.

Complaints

If you believe your privacy rights have been violated, you may file a complaint with our Privacy Officer or with the U.S. Department of Health and Human Services. You will not be retaliated against for filing a complaint.
